Appointment Prep Kit

Medical appointments are your time to tell your health care provider how you’re feeling, both physically and emotionally. It is also your time to discuss your treatment plan – what options you have, whether your current treatment is working, and what steps to take if it is not.

Although you may only have a few minutes with your provider, there are several things you can do to get the most from your time together. Preparing for your appointment helps you work with your health care provider to make the best treatment decision and, ultimately, live your healthiest life with psoriatic disease.

Request your Appointment Preparation Kit to receive:

  • A checklist of what to bring to your appointment
  • Tips on how to best talk to your health care provider
  • Information on treatment types and options
  • Recommendations for telehealth visits

(An NPF Patient Navigator will email your resource(s) to you within 1-3 business days.)
