Hotel information

The National Psoriasis Foundation will provide private accommodations for you on Friday and Saturday, Nov. 13-14, 2009, at the Embassy Suites, Chicago Downtown Lakefront. 511 North Columbus Drive, Chicago, Ill. 60611.

Cancellation policy
Deadline for hotel cancellation is Oct. 27, 2009. Airline tickets are non-transferable and non-refundable, and you will be charged for your airline ticket if you do not use it.

If you register for the meeting and do not attend, your credit card will be charged for your airfare and two night stay at the hotel. No exceptions.

Early hotel departure
In the event you need to check out before Nov. 15, 2009, you will need to let the hotel know before you check in. If you do not notify the hotel prior to check-in, an early departure fee of $50 will be charged to your credit card.

Transportation information

The Foundation provides round-trip coach class airfare to and from Chicago O’Hare International Airport. To begin planning your transportation, e-mail the Foundation travel agent, Linda Miller, at linda_nobhill@yahoo.com.

Your e-mail to Linda Miller should contain:

  • Your name (as it appears on your driver’s license), university name and city
  • Airport of departure/return
  • Preferred time/date departure/return

E-mailing Linda Miller is the preferred method of communication. Linda will send you preliminary schedules to review within a day or two of receiving your e-mail. If you need to talk to Linda directly, you may call her toll free at 877.227.3530. If Linda is not available, please ask for Barb Stavig-Doane.

You are responsible for your own ground transportation.
